Community Brain Injury Services Providence Care – Brockville
Community Brain Injury Services (CBIS) provides community-based services to adults with acquired brain injury across Southeast Ontario, serving residents in the counties of Frontenac, Lennox and Addington, Hastings, Prince Edward, Lanark, and Leeds and Grenville. CBIS offers a variety of programming and services, including group activities in Kingston, Belleville, and Brockville, and operates from offices in all three locations.
CBIS uses a participate-to-learn model, grounded in real-life contexts, where roles are used as goals and participation is supported through personal and environmental supports.
Types of Services
Referrals
Referrals are accepted from service providers, individuals, and family members. A written referral form with medical documentation of an acquired brain injury is required. Forms are available through CBIS offices in Kingston, Belleville, or Brockville. CBIS contacts individuals within one week of referral. Service start dates depend on needs and available resources.

Fees
Services are subsidized by the Ministry of Health and Long-Term Care. Fees may apply for individuals with access to private rehabilitation funding.
Outreach Services
CBIS Outreach provides neurobehavioral rehabilitation addressing cognitive, emotional, behavioural, physical, and pre-vocational needs. Services are designed for adults with moderate to severe acquired brain injury who are able and willing to work toward identified goals. Programming includes individual and group activities in office, home, and community settings.
Assisted Living Program
Provides ongoing (up to 24-hour) support for adults with severe acquired brain injury living in the community. Participants live in their own homes or apartments and receive specialized supports to maintain housing, daily living skills, and community participation. Clients are responsible for basic living expenses such as accommodation, food, and transportation.
Psychology Services
CBIS Psychology Services include assessment (not diagnostic), counselling, consultation, and clinical direction for client services. These services are accessed through the CBIS referral process.
System Navigation / Education
CBIS provides education and resources to community partners, service providers, and individuals, along with service linkages for people living with acquired brain injury (both traumatic and non-traumatic).
Short-term System Navigation and education are available for individuals with medically documented mild traumatic brain injury (mTBI/concussion). Supports may be provided in a one-to-one format or through the Post-Concussion Action Group, a structured series of four sessions. Services are offered in person at CBIS offices, with virtual connections available across Southeast Ontario.
